October 17, 1921 The State Insurance Fund of Utah, State Capital, Salt Lake City, Utah Gentlemen: The Carnegie Museum is in receipt of a letter form Mr. C.F. McShane, Commissioner of the Industrial Commission of Utah, informing us that it is necessary to take out Workmen's Compensation Insurance to cover the three men working in the Dinosaur Quarry near Jensen, Utah. These men are under the direction of Mr. Earl Douglass of this Institution. Will you be good enough to let me know the proper procedure for taking out such insurance and the cost of the same, so that I may at once comply with the law of your state.
