We have your favor of Oct. 17th regarding Compensation Coverage in this State, and if you will fill out and return the enclosed Application we will take great pleasure in issuing policy at once under the following classification and rate:



No. 1622 - Quarrries with or without blasting (N.O.C.) Payroll for Comp. Coverage to include Drivers, Chauffeurs and their helpers -- $4.39



Under the schedule it will be necessary to give the total number of employees and your anticipated payroll expenditure from now until Dec. 31st, 1921, at which time the policy will automatically renew for further periods of six months upon new estimates.  It will be necessary to include the salaries of all employees upon the ground, even though some are directing the procedure
